Ratios: 1:18 days Certifications: Will accept BSW, MSW, LMSW, or LCSW Skills: Discharge Planning, Conduct initial evaluation, Develop plan of care to meet patient/caregiver needs, Development of problem list, Establish and revise goals, Coordinate services to home care and equipment following discharge, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to other disciplines/resources, Refers to rehab and long term care facilities, Discharge Summary, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation reflects understanding of coverage criteria, Documents patient response, Medicaid, Medicare Years of experience REQ: 1 First-timers accepted: (Y/N) Yes Certs REQ: BLS Is on-call REQ?
